Vmax Vision announces the addition of voice-guided instruction software to the PSF Refractor, its proprietary subjective refraction technology. The system uses Point Spread Function principles to deliver more accurate and reliable refraction results.

The voice instruction software is the first of its kind, audibly guiding the patient through each refraction test. With the addition of this unique voice instruction, refraction training time is significantly reduced, while also ensuring a consistent quality of testing.

According to Vmax Vision, the decrease in refraction training time brings direct profits to the practice. New doctors can more quickly master the refraction process and begin treating patients. Supra Scan Photocoagulator Platform Is Customizable

FCI: A First in Nascolacrimal Intubation Stents
FCI Ophthalmics has introduced what it calls the first ever “pushed” monocanalicular nasolacrimal intubation stent, the Masterka. Designed by oculoplastic surgeon Bruno Fayet, MD, Paris, France, the Masterka offers a safer and faster intubation of tear ducts as it does not require the frequently difficult step of recovery inside the nasal cavity. 

The Masterka is indicated for congenital nasolacrimal duct obstructions that are resistant to probing. Unlike the traditional “pulled” technique, in which the stent is advanced through the nasolacrimal system and retrieved through the nose by pulling on the guide probe or thread, the Masterka has no metallic probe or suture attached to it and, therefore, it is not pulled out of the nose. Instead, the Masterka is pushed into the nasolacrimal duct and anchored in place at the punctum by a plug-like fixation head similar to FCI’s signature Monoka stents.

 
A pushed intubation is more similar to a simple probing than a pulled intubation and can safely permit the use of a face mask with less risk of airway compromise, thus resulting in significantly reduced time for the intubation procedure, the company says.

The Masterka device consists of a silicone tube molded to a fixation head and pre-mounted on an introducer to facilitate insertion. The introducer is easily and completely removed once the intubation of the lacrimal passages has been completed. The Masterka comes in 30, 35 or 40 mm lengths and must be selected according to each patient’s needs. Special sizing devices are sold separately. 

The insertion of the Masterka device using the push technique is performed in the same manner as a nasolacrimal duct probing and the insertion of the traditional stents into the nasolacrimal duct system. A major difference is that the introducer for the Masterka is retrieved by withdrawing it upwards and back out through the canaliculus and punctum, thus avoiding the sometimes traumatic nasal recovery step of pulled intubations. After insertion, the seating of the Masterka anchoring fixation head is performed in exactly the same manner as all of the other devices in FCI’s Monoka family of intubation devices. An Allergan-funded study assessed the company’s new formulation of bimatoprost 0.01% with a lower concentration of the preservative benzalkonium chloride (0.02% BAK), designed to maintain the intraocular pressure-lowering efficacy of earlier bimatoprost 0.03% and provide an improved ocular surface tolerability profile.
The three-month, multicenter, randomized, investigator-masked, parallel-group comparison study included adults diagnosed with open-angle glaucoma or ocular hypertension. Exclusions included: use of bimatoprost or travoprost within six months, any corneal abnormalities preventing IOP assessments, and a history of refractive surgery or punctal plug use.
All patients underwent a 30-day run-in on latanoprost (Xalatan) monotherapy. At the baseline visit, 164 patients discontinued latanoprost monotherapy and were randomized in a 1:1:1 ratio to either: bimatoprost 0.01% preserved with 0.02% BAK (Lumigan); travoprost 0.004% preserved with sofZia (Travatan Z); or latanoprost 0.005% preserved with 0.02% BAK (Xalatan).
The key findings by outcomes included:
  •  Hyperemia. There were no significant differences in conjunctival hyperemia scores among groups at baseline. At week one, there was a statistically significant among-group difference in change from baseline in conjunctival hyperemia (0.04 for bimatoprost, 0.20 for travoprost, and 0.00 for latanoprost; p=0.018); pairwise t tests were significant for travoprost vs. both bimatoprost and latanoprost, but not for bimatoprost vs. latanoprost. Changes within group were not significant at any visit for each of the bimatoprost and the latanoprost groups; however, with the travoprost group, change from baseline was significant at weeks one and four where the mean change (standard deviation) was an increase of 0.20 (0.39) at week one (p<0.001) and an increase of 0.13 (0.47) at week four (p=0.046).
  • Corneal staining. There were no significant among-group differences in mean corneal staining at baseline or change from baseline at any visit (p≥0.379).
  • Tear breakup time. There were no statistically significant among-group differences in mean TBUT scores at baseline or at any visit, nor were there any significant among-group differences in change from baseline at any visit (p≥0.630).
  • Biomicroscopy. There was a ≥1-unit increase in hyperemia in 2 percent of the bimatoprost group, 5.9 percent in the travoprost group, and 3.8 percent in the latanoprost group. There was a ≥1-unit increase in corneal staining in 2 percent of the bimatoprost group, 5.9 percent in the travoprost group, and 3.8 percent in the latanoprost group. There was either no change or a decrease in conjunctival hyperemia in 88.2 percent of patients receiving bimatoprost, 70.6 percent of patients receiving travoprost, and 84.6 percent of patients receiving latanoprost at week 12.
  • Intraocular pressure. IOP was not significantly different among the groups at baseline. There were no significant among-group differences in change from baseline at any follow-up visit. Bimatoprost demonstrated a statistically significant reduction in IOP from baseline at every follow-up visit. There were no statistically significant reductions from baseline in the travoprost group at either week one (p=0.056) or week four (p=0.138). Change in IOP was significant only at week 12 for the travoprost group. There were no statistically significant changes from baseline in the latanoprost group at any visit (p≥0.239).
The researchers conclude that at 12 weeks, patients had comparable ocular tolerability findings with respect to objective measures of conjunctival hyperemia, corneal staining and TBUT. Differences in type of preservative (i.e., BAK vs. sofZia) did not lead to differences in ocular surface tolerability. Adverse event incidence rates were comparable among groups.

Bioptigen Gains Clearance to Market Envisu Spectral Domain Imaging System
Bioptigen Inc. has received 510(k) clearance from the Food and Drug Administration to begin marketing its hand-held Envisu Spectral Domain Ophthalmic Imaging System devices for patient use. The Envisu SDOIS aids in the diagnosis of physiological and pathological conditions of the eye through non-contact optical imaging. In receiving this regulatory clearance, Bioptigen’s Envisu SDOIS become the first optical coherence tomography imaging systems commercially available in the United States for hand-held and pediatric imaging.

Envisu’s ergonomic, hand-held scanner enables imaging of patients of all ages—from premature and neonatal infants to adults—whether they are upright or supine, ambulatory or confined. And interchangeable lenses allow clinicians to image various structures of the eye, from cornea to retina.
Bioptigen already has received regulatory approval from Canadian, European and Australian authorities to commercially market its Envisu C2200 and C2300 ophthalmic imaging devices.
