Self-Assessment Directions
To obtain a certificate of completion for 4 hours of Category 1 CME credit toward the AMA Physician’s Recognition Award, you must successfully complete the exam. A minimum of 70% correct is required to obtain a certificate of completion.

You may also fax your answer sheet to (212) 219-7849. There is a maximum four week processing time on all submissions received.

Downloading the Examination Answer Sheet
To download the Examination Answer Sheet, click on the Examination Answer Sheet link below.

• If you are using the Microsoft Internet Explorer browser, the .PDF file will automatically launch. Go to the File menu at the top left of your screen, and select Print. When the printer window opens, simply click OK.
  
  
• If you are using a Netscape browser, you may be asked whether you want to Open the file or Save the file to a disk. Select the Save It option. In the Save In: field, select your computer desktop, then click the Save button. Once the file has downloaded to your desktop, double-click on it to open it. In Acrobat Reader, click on the File menu in the upper left-hand corner, then click Print. When the printer window opens, simply click OK.

If you do not have the Adobe Acrobat Reader
If the Adobe Acrobat Reader is not installed on your computer, you can download it free of charge from the Adobe Web site. Simply click on the Acrobat Reader icon below, and follow the installation instructions on that page. Once you've installed the program on your computer, return to this page to download the Examination Answer Sheet.
